A tooth that's been aching for weeks, a molar that cracked on a popcorn kernel, a baby tooth that won't budge while the adult tooth comes in behind it — these are the kinds of situations that bring people to our office in Woodhaven asking whether a tooth needs to come out. Most of the time, our goal is to save a tooth with a filling, crown, or root canal. But when a tooth is too damaged, too decayed, too loose from gum disease, or simply in the way of healthy teeth, removing it is the step that protects everything around it.
An extraction is the removal of a tooth from its socket. We perform two kinds, and the difference comes down to how the tooth is positioned and how much of it is accessible above the gum line. A simple extraction is used when the tooth is fully erupted and can be loosened and lifted out with hand instruments. That covers many badly decayed teeth, teeth broken down from old failing restorations, loose teeth affected by advanced gum disease, and baby teeth that are hanging on too long. It's done with local anesthetic, and it's usually quicker than people expect.
A surgical extraction is used when a tooth isn't fully accessible — it may be impacted beneath the gum or bone, broken off at the gum line, or have curved roots that need to be sectioned and removed in pieces. Wisdom teeth are the most common example, though a fractured molar or a tooth that never erupted can call for the same approach. A surgical extraction involves a small incision and sometimes a few stitches, and it generally takes a little longer to heal than a simple one. We offer both here, so you don't have to be sent elsewhere to finish treatment, and we accept patients for oral surgery without a referral.
Before anything is scheduled, we examine the tooth, take X-rays, and talk through whether it can realistically be saved. If it can, we'll tell you — root canal therapy, crowns, and fillings are all part of what we do. If it can't, we explain exactly which type of extraction is needed and why, and what your options are for replacing the tooth afterward.
Comfort tends to be the biggest concern patients bring up, especially for surgical cases. Local anesthetic keeps the area numb throughout. For patients who feel anxious, we offer nitrous oxide and IV sedation, and our comfort menu — blankets, warm towels, neck pillows, headphones, Netflix, and refreshments — is available during any visit. We see all ages, including children from age one through the teen years and children with special healthcare needs, so a nervous ten-year-old and an apprehensive adult both get the same unhurried approach.
Things worth telling us ahead of time:
- Any medications you take, including blood thinners
- Medical conditions or recent surgeries
- Past reactions to anesthetic or sedation
- Whether you've had trouble healing after previous dental work
- If you'd like sedation, so we can arrange a ride home for you
Healing, Aftercare, and What Comes Next
A blood clot forms in the socket right after the tooth is out, and protecting that clot is the main job of the first day or two. We send you home with specific instructions — how long to bite on gauze, what to eat, when to resume rinsing, and why to avoid straws and smoking early on. Soreness and some swelling are normal, and we tell you what's expected and what warrants a call. If stitches were placed, we let you know whether they dissolve or need removal.
Removing a tooth often isn't the end of the plan. An empty space can let neighboring teeth drift and bone shrink over time, so we talk about replacement early. Depending on the situation that may mean a bridge, a partial or complete denture, or a dental implant, sometimes with bone grafting to preserve the ridge at the time of extraction. Our on-site dental lab makes and adjusts crowns, dentures, night guards, and repairs here, including same-day dentures and crowns, which keeps follow-up visits efficient.
